Phase I - Breast Center Type and Select Profile Characteristics

Breast Centers will be asked seven profile questions.  All seven must be answered.  The responses will allow centers to compare themselves with like centers, in the reports being offered, by choosing a combination of these filters.  The following gives you an idea of the seven questions.
 

1.  You will be asked to identify your Breast Center's Components of Service from the following table.  Your responses will determine your type of "Quality" center and the type of center as you are as viewed by your patients.  This is based upon services being either provided or referred and whether or not the center has quality influence on the service.

Example:   XYZ Breast Center provides input. Based upon the responses it is determined that
According to the services XYZ center shows as being PROVIDED by the XYZ center, XYZ center is eligible to become a Certified Screening Breast Center ; and, according to the services XYZ center shows as  being PROVIDED AND/OR REFERRED, XYZ center, as viewed by patients, is a Diagnostic and Treatment Breast Center.

2.  To identify your regional and district location
      you will be asked to identify the state in which your center is located.

3.  To identify the location of your patient population
      you will choose from rural, suburban,  urban, metropolitan

4.  You will be asked to identify the volume of imaging patients served
      Number of Mammograms performed for the most current year selected
      Screening, Diagnostic and Total Screening and Diagnostic

5.  You will be asked to identify the number of new breast cancer patients      
      treated or diagnosed for the most current year selected

6.  You will be asked to identify your Patient Population Ethnicity

7.  You will be asked to select the choice that best describes your center's ownership, administration and oversight
