Step 1 – Complete and submit the participation interest form contained on the NCBC web site.

Step 2 – Just print, review, sign and fax the Data Validity Agreement to the NCBC office.

three easy steps to access quality measures

Step 1 – Upon receipt of your Data Validity Agreement, a representative of the NCBC will send you an email with your Quality Initiative ID participation number.  This number allows you access to the National Quality Measures for Breast Centers™ on the NCBC web site.

 Step 2 – Review and agree to the End User Licensing Agreement on the internet. (Click on the Quality Button on the NCBC web site to enter Quality Section and then click on the Access Button.)

 Step 3 – Access the data input forms to begin your participation in the National Quality Measures for Breast Centers™.
